What you'll be working: Our standard working hours for this role are 37.5 hours per week, Monday to Friday. What you'll be doing: Keep people safe from harm and protect their human rights Act as care co-ordinator in line with Active Care Group’s standards Develop and deliver evidence-based packages of care including specific psychosocial interventions where appropriate Undertake carers assessment and care planning Maintain health and social care records Lead the provision of therapeutic group work as indicated by the needs of our young people Provision of case reports when required (e.g., for court, mental health review tribunals / managers hearings, case reviews, enquiries, etc) and attending case discussions, conferences, and reviews as necessary including those integral to child protection procedures Provide patients, relatives and carers with information and support regarding entitlements to statutory benefits Adhere to the policies and procedures of the organisation, with particular regard to confidentiality, financial and administrative responsibilities, and treat residents, colleagues and visitors with dignity and respect at all times.
