We are an innovative, integrated Community Nursing service covering the Lichfield and Burntwood area. We work closely across the Nursing teams, including our Community Rapid Intervention Service, Unplanned Care, Podiatry, Physio, Falls Team, Stroke and Rehab teams whilst also liaising with our out of hours service and our Palliative Care team and Local Hospices to ensure that our patients are at the centre of the care provided and have access to the services they need when they need them.
