Clinical Responsibilities Work within a Caseload Management Framework to coordinate care using clinical judgement and partnership with the wider Primary Health Care Team. Identify and plan preventative and anticipatory care needs. Maintain accurate, contemporaneous patient records and update relevant professionals on changes in condition. Proactively monitor patients and review care plans regularly. Make referrals, request diagnostic tests and initiate treatments or interventions as required. Assess patients with complex needs using advanced clinical examination, history taking and evaluation skills. Manage End of Life Care in partnership with health, social care and voluntary agencies. Prepare and support patients and families for changes in condition and endoflife choices. Organisational Responsibilities Work in accordance with EPUT policies, procedures and guidelines. Ensure timely reporting of accidents, incidents and near misses. Sign off incidents on DATIX within 10 working days, following Trust policy. Submit statistical and contractual monitoring returns within agreed timescales. Ensure Mandatory and Statutory Training is completed within required timeframes. Manage annual leave in line with Trust policy and SOPs. Participate in clinical supervision. Professional Responsibilities Continuously review and update practice in line with Trust policy. Work in accordance with the NMC Code and maintain professional accountability. Maintain NMC registration and keep professionally updated. Prescribe safely and appropriately if holding a recognised Nurse Prescriber qualification. Participate in and lead audit and research activities when required. Uphold confidentiality standards and ensure junior staff understand their responsibilities. Participate in supervision and appraisal processes, ensuring staff receive regular 1:1 support. Provide patients, carers and families with information to support informed decisionmaking. Identify community health needs and contribute to health education and prevention programmes. Work collaboratively with Neighbourhood Teams and partner agencies to deliver care within available resources. Prioritise and coordinate multiple healthcare needs for patients. Develop personalised care plans with patients/carers, including Person Held Records for complex cases. Communication Responsibilities Maintain effective communication and liaison with Neighbourhood Teams, social care, acute trusts, voluntary agencies, patients, families and carers. Coordinate communication within primary health care teams, including regular multidisciplinary meetings. Provide highquality written reports and documentation as required. Participate in and lead professional groups developing policies, protocols, standards and audits. Lead clinical audit to determine standards of practice and use research awareness to evaluate effectiveness. Provide education, support and training for staff and students, including pre and postregistration nurses. Access relevant training opportunities for personal and professional development. Ensure team members access appropriate training to manage caseloads effectively. Leadership and Management Responsibilities Provide leadership and judgement in managing the Community Nursing Team to maintain high standards of care. Manage delegated budgets where appropriate. Oversee staff performance and identify training and development needs. Participate in the Trust appraisal process. Ensure all team members work in accordance with Trust policies and procedures. Participate in recruitment and selection processes when required. Manage staff who do not meet job description requirements. Liaise with other Team Leaders to ensure equitable staffing and workload distribution across the service.
