Crynodeb
To be responsible along with other members of the team for the implementation of interventions as outlined in service users care plans. Attending to the physical and psychological needs of the children and young people under the care of the nursing team. Act as an associate worker for service users. To take support the service in delivering the pathways promoted by the service including Mental Health and Challenging Behaviour. Ensure that accurate and objective information is recorded and signed by the appropriate member of staff. Establish therapeutic relationships with individual children and young people, relatives and carers are maintained by the nursing team. Contribute towards the formulation of care. Ensure effective and confidential communication at all times maintaining strict confidentiality with regard to any information concerning patients and colleagues. Demonstrate an awareness of the needs of carers of patients with learning disability. Be aware of the Mental Capacity Act and Mental Health Act and how to implement this safely. Be involved and contribute to the planning of each childs care in accordance with agreed pathways, Building the Right Support and the Learning Disability Improvement Standards. Observe any changes in a childs physical condition or mental state and document and report in an appropriate manner. Act in a relaxed manner, thereby contributing to the creation and maintenance of a calm and relaxed atmosphere/environment. Establish positive professional and appropriate relationships with patients adopting a non-judgemental approach. Always be prepared to listen and treat each child with respect and courtesy; valuing them as individuals. To participate in managerial supervision. Develop networking opportunities with other stakeholders to support care in the community and where hospital admission is required to support timely discharge. Develop networking opportunities with other mainstream mental health providers including inpatients; acute hospital and community services to support them to make their services accessible to children and young people with LD.
