Community Support Worker

What You’ll Be Doing

As a Community Support Worker, you will:

Improve outcomes for adults, children and young people

Support individuals to build independence, confidence and life skills

Be involved in team meetings to review and improve the support provided

Work collaboratively with families and multidisciplinary professionals

Follow person-centred plans designed around individual goals

Brain Injury

Neurological Conditions

Learning Disabilities

Mental Health

Physical Disabilities

Spinal Injuries

Complex and long-term conditions
