We have an exciting opportunity for a motivated and enthusiastic individual to join the Adult Contact team in the south in a 30 hour a week role as a social care assessor. This post is an exciting opportunity which will primarily focus upon supporting individuals where hoarding is identified.

About the role
We are looking for an SCA who is dynamic and able to build links and relationships to improve and support a person through what can be a difficult and challenging experience in their lives and to enable them to become more in control of their life choices. The post is within a busy frontline adult social care team and will be hybrid working.

The nature of the work can be intensive and prolonged but has the potential for positive outcomes for both the person and worker, it supports a person to be empowered to take back control over their lives and circumstances.

About you
What we are looking for:

We are looking for an enthusiastic Social Care Assessor who understands adult social care, mental capacity and safeguarding issues.
The candidate should be self-motivated with excellent communication skills and the ability to develop and work both autonomously within the Adult Contact team and the wider multi-disciplinary arena.
You will need to have good organisational and time management skills as this is key to effectively managing caseloads.
The candidate should understand asset and strengths-based support and be able to remain calm in a variety of situations.
It will be essential for the post holder to work autonomously within the Council’s policies and professional practice, in addition to maintaining accurate and comprehensive client documentation.
