This is an exciting opportunity to join Cheshire East as part of Adult social care. We are looking for two motivated and enthusiastic individuals to join our Adult Contact team as social care assessors on a 12 month contract. Your work will have a Prevent Reduce Enable focus supporting individuals with care and support needs and their carers. Based at Delamere house you will be part of a busy frontline social care team.

About the Role:

You will carry out basic needs-based assessments with vulnerable people and their carers within agreed procedures, to determine specific individual client needs, identify levels of risk and urgency of response in order to construct an individual Care Plan.
The post will provide a wide range of learning and training opportunities and includes joint working across the Health and Social Care sector. It will provide you with an excellent opportunity to progress your career and will offer you a wide range of experiences within Adult Social Care services

About you:

We are looking for individuals with

Experience of working with vulnerable people and their carers in a Social Care or Health setting.
NVQ3, or equivalent experience in Social Care
Knowledge of assessment and care management process, including how to understake needs based assessments.
