• To undertake specific support work to aid rehabilitation of service users, in the achievement of optimal functional independence, with or without the assistance of others.

• To provide flexible programmes and interventions of therapeutic activities, as directed by a registered member of staff

• To work without direct supervision, monitoring progress and minor adaptations to interventions as required, under the guidance of a registered member of staff.

• To communicate effectively and work collaboratively with all members of the multidisciplinary team and other services within Home1st, as well as outside agencies.

• Working within any of the Home 1st Planned Care Therapy pathways and participate in rotation to different therapy teams as required.

• Participate in development of integrated pathways.

• To be responsible for the ordering and provision of Allied Health Professional or Band 4 Rehabilitation Assistant prescribed equipment.

CLINICAL DUTIES
• To provide physiotherapy, occupational therapy and nursing interventions as part of a specific treatment plan agreed and documented by physiotherapist/occupational therapist/nurse.
• To undertake tasks in delivering patient care, rehabilitation or social care as directed by the service professionals in a range of settings.
• To work independently in the designated clinical area/patients home following an agreed treatment plan.
• To undertake support work with patients individually or in a group with or without supervision of a physiotherapist or occupational therapist.
• To follow departmental and trust guidelines for obtaining consent and use diplomacy and gentle encouragement to enhance patient’s compliance in treatment programmes.
• To work with family members, carers and support workers to enable them to carry out treatment programmes planned by the physiotherapist, occupational therapist or nurse.
• Manage own workload allocated by the physiotherapist, occupational therapist or nurse
• To work as a member of the multidisciplinary team and liaise with the team effectively to ensure co-ordinated approach to patient care and attend meetings as delegated by the nurse, physiotherapist or occupational therapist.
• Maintain comprehensive and accurate records of work undertaken, including documenting in line with Trust and service standards.
• To attend in-service training, clinical supervision and team meetings as required
• To assist physiotherapist, occupational therapist or nurse with complex patients in their own home environment.
• Have the ability to handle patients with mobility problems, in an enabling way. This may include the use of hoists, sliding sheets, wheelchairs, mobility aids etc.
• To actively participate within the completion of Band 3 level competencies.

EFFORT

• To carry out interventions with patients with a variety of conditions, as directed by the AHP, with frequent episodes of low to moderate physical effort, with occasional episodes of moderate to intense physical effort on a daily basis e.g. transferring patients, use of hoists, therapeutic handling technique positions and transporting of patients in wheelchairs.

• To comply with the CNWL-MK Manual Handling Policy and local therapeutic handling guidance at all times. Work may involve working within cramped conditions, such as next to the patient’s bed. Physical requirements include: repetitive movements, standing / walking for the majority of the working day (intermittently involving bending, crouching, kneeling, twisting and crawling).

• Within limits of skills and experience, to deal sensitively with patients, and carers, who may have high levels of anxiety, fear or aggression due to pain, limited mobility, frustration, terminal illness, dementia or neurological deficits, frequently on a daily basis. Although not acceptable, these patients may use verbal or physical aggression towards staff. All such incidents must be reported to the appropriate person in line with CNWL-MK policies. Support and advice, if needed, in this event is available through Trust networks and Line Managers.

• The job involves frequent exposure to unpleasant working conditions on a regular (daily) basis, e.g. bodily fluids including sputum, faeces, vomit, urine, blood and contagious conditions, and occasional exposure to unsanitary home conditions.

• To use a moderate level of mental effort and concentration frequently during the day whilst assessing and treating patients, and fulfilling the documentation requirements of the job.

• To use emotional effort in the development of a therapeutic relationship with patients in difficult circumstances.

• To come into occasional contact, and be able to interact appropriately with patients and relatives who may be experiencing depression, difficult family circumstances or poor prognosis, and to facilitate intervention, as directed by the Clinician, through empathetic and sensitive use of motivational skills.
ORGANISATIONAL DUTIES
• Maintain equipment identifying defects, hazards and equipment failure and notifying the relevant manager
• Manage own caseload and work only within own scope of competence.
• To maintain up to date electronic diary and outcomes of all appointments as required by the service
• To maintain the safety of designated clinical environment according to local policy and regulations and aware about the emergency procedures.
• To assist in risk assessments and comply with trust policies on incident reporting
• To adhere to trust appraisal and performance review programme to identify personal development plans and training needs.
• To participate in data collection and statistics as directed
• To attend mandatory and relevant training as and when required

GENERAL DUTIES & RESPONSIBILITIES
• To assist in simple administrative procedures as required by the Team Lead as part
of everyday activity
• Ensure that there is effective communication with the nurse, physiotherapist or
occupational therapist regarding treatment of individual patients
• Comply with the organisational and departmental policies and procedures.
• Administration responsibilities include all general office tasks, such as photocopying,
computer data input, filing and answering the telephone in a polite and courteous
manner.
• To maintain accurate and comprehensive clinical records and all documentation
processes (e.g. reporting incidents and near misses and clinical notes).

PERSONAL RESPONSIBILITY (ORGANISATIONAL EXPECTATIONS)

In order to fulfill the job responsibilities and the environment in which these responsibilities are carried out the job holder is required to:

1. Maintain the highest standards of care and service, taking responsibility not only for the care you personally provide, but also for your wider contribution to the aims of your team and the NHS as a whole.

2. Be aware of and follow at all times the relevant National and Local codes of practice in relation to their role and function. If you are in a post that requires registration with a professional body, you are required to maintain that registration with the appropriate professional body.

3. Protect the confidentiality of personal information that you hold unless to do so would put any one at risk of significant harm. Keep accurate and professional records and information about staff, patients and clinical care, using the document based and computer based systems in operation in the Trust. Work in accordance with local policies and procedures and the statutory frameworks, which govern confidentiality and data protection, information sharing and disclosure.

4. Take reasonable care of Health and Safety at work for you, your team and others; ensure compliance with health and safety standards and legislation; attend all relevant Health and Safety mandatory training.

5. Take responsibility for attending and participating in all mandatory and essential training to ensure the safe and efficient functioning of the Trust and/or safety and wellbeing of other staff and the patients for whom you provide services.

6. Participate in the appraisal process on an annual basis, as a minimum, in accordance with the Personal Development Review Policy to explore and identify development needs to ensure that you are able to fulfil your job role and meet all objectives set through the process.

7. Be familiar with and comply with, Trust policies for infection prevention and control and hand hygiene in order to reduce the spread of healthcare-associated infections. This will include a requirement to attend mandatory training in Infection Prevention and Control and to be compliant with all measures known to be effective in reducing healthcare-associated infections, including correct uniform and dress code policy, the use of personal protective equipment policy, safe procedures for using aseptic techniques, and safe disposal of sharps.

8. Safeguard and promote the welfare of children and vulnerable adults by being aware of and working in accordance with statutory guidance and local policies and procedures and attending training to a level required to ensure that you are competent to fulfil your responsibilities.
