Are you highly organised, proactive, and passionate about supporting people to return home from hospital safely and efficiently?
We’re looking for an exceptional Health & Social Care Co‑ordinator to play a vital role in delivering the Home First approach across Dorset.
This is a critical and evolving role at the heart of a system committed to improving patient flow, reducing delays, and ensuring individuals no longer clinically requiring a hospital bed can transition home or to community-based support in a timely, well‑coordinated manner.
Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.
Employment in this post requires a Standard Disclosure and Barring Service (DBS) check which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
As a key member of our Transfer of Care team, you will:
-Work closely with acute and community teams across Dorset to ensure a seamless journey for people preparing to leave hospital.
-Process referrals efficiently, making sure information is shared promptly and accurately.
-Co‑ordinate Multi‑Disciplinary Team (MDT) meetings, ensuring actions are followed up and communication remains clear and consistent.
-Monitor and escalate safeguarding concerns, ensuring risks are identified and responded to quickly.
-Support system partners by providing information, attending meetings, delivering updates, and progressing agreed actions.
-Use multiple health and social care systems to facilitate effective multi‑agency communication and collaboration.
