Key results from the job holder To carry out assigned tasks involving direct care in support of a Registered Nurse by supporting individuals in the achievement and / or maintenance of their health and wellbeing this can include health promotion for client group. This may also involve (when competencies achieved) blood pressures, blood samples, finger prick technique, urinalysis, bowel care, nutritional support, observations, specimen collection, prevention of pressure ulcers, moving and handling, weight - hoist, wheel chair, height, urinalysis, oxygen statistics, respirations, pulse, waist measurements, -promote personal care skills of the individual Carrying out behavioural and clinical observations as instructed by the registered nurse Support people with learning disabilities to access generic screening services Planning and Organisational Duties To operate within the organisations clinical governance framework Ensure confidentiality is maintained at all times. To adhere to trust policies and procedures. Ensure clear understanding of your own role and responsibilities within sphere of competencies, by influencing the development of own knowledge ideas and work practice. Attend and participate to client focused person centred and multi-disciplinary team meetings. Work in line with the organisational lone worker policy Maintain own skills attend mandatory training. Implement health promotion/ education to 1-1 or a small group as appropriate e.g. smoking healthy living etc Assist in the health promotion and education along with the registered professionals. Communications and Key Working Relationships Work in collaboration with the MDT under the supervision of the Registered Nurse Work independently under a care plan from the registered nurse Communicate findings to the Registered Nurse to assist with assessment and implementation of care to the person with a learning disability. Assist with the identification of need for ongoing referrals and have a knowledge of services available to support with health promotion, wellbeing of the person. Assist the Registered Nurse in establishing and maintaining an effective communication system. To work and communicate appropriately in situations that may be sensitive or contentious. To undertake project work under the supervision of the Registered Nurse. To maintain accurate patient records in accordance with trust guidance and data protection standards. Responsibility for Finance Identify and report disrepair of equipment. Responsibility for Human Resources Identify own need for personal development, attend mandatory training. Responsibility for Health & Safety Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. Responsibility for Teaching Support the induction of new staff Work Circumstances & on-call The service operates a buddy system as part of the lone working policy and local procedures which the post holder will participate in. The work predominantly is organised between the hours of 9-5, but flexibility is required to ensure the needs of the patients are addressed outside these times.
