Summary


We are looking for a highly motivated nurse with experience in caring for patients with palliative care needs and at the end of their life. You will provide advice and support to palliative care patients as part of a supportive and well-established team.

The service working hours are 0800-1800 Monday to Friday and 09.00-17.00 Saturday and Sunday. You will be required to work flexibly to help provide the 7 day service, working weekends on a rotational basis.

The post holder is responsible for the provision of high quality, patient focused care

Facilitating transfer of appropriate patients requiring end of life care to hospice care

Responsible for assessing, developing and implementing specialist nursing care programmes

Responsible for providing specialist advice and support to, and liaising with, other staff
