Crynodeb
Utilising Physical Health assessment skills to deliver direct clinical assessment and care provision of people in their home environment. Co-ordination of daily Multi-Disciplinary Team meetings and Consultant ward rounds, ensuring that patient related information is up to date and relevant clinical information is disseminated clearly and concisely. Act as a point of contact providing advice, support and guidance to patients, colleagues and other healthcare professionals on issues relating to patient care. Communicating sensitive information concerning patients medical condition(s) and status which involves relationship building; displaying persuasive and reassurance skills acting in the best interests of patients at all times.
