Crynodeb
To support patients/service users in undertaking activities of daily living within care plans. Contribute to the assessment and development of care plans under the direction of a Registered Health or Social Care Professional Undertake the role of health promoter and educator for patients/service users and their support networks. Recognise changes in patients'/service users' presentation which require the intervention of others and understand when to refer to the supervising Registered Health or Social Care professional and other colleagues for reassessment of patient care. Report all accidents and incidents as per policy and demonstrate an awareness of risk management and Quality and Safety processes. Report and escalate any safety, safeguarding or risk concerns to the appropriate member of staff and ensure that action is taken.
