Job Title: Practice Nurse
Location: Leeds, West Yorkshire
Salary: Band 5–6 equivalent (depending on experience)
Hours: Full-time or part-time considered
Contract: Permanent
About the Role
We are looking for an enthusiastic, clinically skilled Practice Nurse to join our friendly and supportive general practice team in Leeds. The ideal candidate will be experienced in providing high-quality, patient-centred nursing care and have a proactive approach to health promotion, long-term condition management and preventative care.
You will work as part of a multidisciplinary team providing safe, effective and accessible services for our patient population.
Key Responsibilities
Treatment Room Skills-
Wound care and dressings
-
Ear irrigation
-
Cervical cytology
-
Childhood and adult immunisations
-
Travel health advice and vaccines
-
Long-term condition reviews where appropriate
-
ECGs and blood pressure monitoring
-
Phlebotomy (if required)
Chronic Disease ManagementExperience in one or more of the following is desirable:

Asthma reviews
-
COPD management
-
Diabetes care
-
Hypertension management
-
Heart disease and stroke prevention
-
NHS Health Checks
Professional & Clinical Duties-
Deliver evidence-based care following national and local guidelines
-
Assist in the management of clinical recall, screening and QOF delivery
-
Participate in safeguarding, infection control and clinical governance
-
Maintain accurate, up-to-date clinical records
-
Support health promotion activities within the practice
-
Work collaboratively with GPs, ANPs, HCAs, clinical pharmacists, social prescribers and wider PCN staff
Person Specification
Essential-
Registered Nurse (NMC registration)
-
Experience in a primary care or community setting
-
Competent in immunisations, cytology and wound care
-
Excellent communication and organisational skills
-
Ability to work both independently and as part of a team
-
Confident using clinical IT systems (training can be provided)
Desirable-
Asthma or COPD diploma (Level 6)
-
Diabetes management qualification
-
Experience delivering NHS Health Checks
-
Experience working within a GP practice
-
Ability to assist with QOF, audits, and clinical recall systems
