Health Mentor - 28 hours - fixed term 12 months

Our Healthy Child Programme 0-19 (25 SEND) Service for Health Visiting and School Nursing has a vision to improve health outcomes for our population and influence the wider determinants of health, through collaboration with the people of Tameside and Glossop and our health and care partners. We are currently progressing through an exciting service transformation in conjunction with all of our team.

We are seeking an individual to join our Health Mentor Team, within our School Nursing Service. This role is an exciting and unique opportunity to work with children and young people to support with their health and wellbeing. Providing education and brief in interventions n a range of public health topics.

We work in neighbourhood models which align with these other services to best meet the needs for the community. Our staff teams are welcoming, friendly supportive and innovative, encompassing and valuing all our team members. Our visible Leadership team is positive in driving service improvement in a compassionate and collaborative way working with all teams to ensure we deliver high quality care in a supportive team environment.


The Health Mentor role involves the provision of early intervention with children, young People and families where there is a potential risk to health and wellbeing. This role will focus on key lifestyle issues that have an adverse impact on health and wellbeing-
- Alcohol/Drugs
- Smoking
- Healthy Weight
- Oral Health
- Sexual Health including teenage conception
- Emotional Health and Wellbeing
- School Readiness and transition to Primary School
- School transition to high school
- Transition to Adult Services

The work involves working within the School Nursing Team to support children and young people on these health needs, providing advice and signposting them. Providing packages of care and at times 1-1 support within schools across Tameside.
