Summary
The Carers Intensive Support Team are looking for a hard working, dedicated and compassionate individual to join us in supporting the health and wellbeing of those caring for a person with suspected or confirmed dementia, frailty, or an older person with mental health concerns.
You would be joining a unique team dedicated to supporting family carers responding to crisis and to look after their physical, mental, emotional and social health to enable them to maintain their caring role and build resilience for the future.
You will need to be willing to work across Thurrock Council Area, to attend Carers Groups and Events as well as cover annual leave and sickness absence
You will need a valid Full UK Driving Licence and access to a car as the role requires travel across South Essex.
Working with Dementia & Older People’s Mental Health Team, Frailty Team and Community partners across Thurrock to identify and support carerswhose physical and mental health is deteriorating due to deconditioning and the strain of caring. (Carersof any age caring for a person with suspected or confirmed dementia or a carer 65 and over caring for someone who has mental health concerns or who is frail aged 65 or over in the most complex caring situations with time limited interventions to achieve their optimal goals)
Based in Thurrock OPMH Team and working with Frail + partners using FrEDA (Frailty, End of Life & Dementia Assessment) and the Carers Health & Wellbeing Check a personalised care and support planning tool linking the PCNs, Community teams and Acute partners through S1
You would identify and support opportunities for Carers Health Checks to
· Ensure early identification of respiratory concerns.
· Ensure carers uptake of flu, pneumonia and other vaccinations to prevent hospital admissions and ability to maintain caring role
· Ensure opportunities to discuss lifestyle choices such as smoking, diet, inactivity, that may contribute to increased risk of cancer
· Undertake or refer for BP, Pulse and other physiological monitoring
· Early identification of Atrial Fibrillation and lifestyle advice to ensure early identification of hypertension concerns and education

• To identify rising carer burden in older carers and take action before carer breakdown
• Identify rising carer burden and support those carers who are disadvantaged either by literacy deficits or digitally to access all available support to build understanding and resilience for the future
• Support those to access help if they are facing fuel and/or income poverty, digital and literacy exclusion and deprivation
• Support carers to prevent or significantly reduce readmission to acute hospital of cared for older adults who may have complex Long Term Conditions, are Frail, have mental health problems and/or have Dementia or those at End of Life.
• To walk alongside carers and be their tangible link between all the services to pull together the optimal individualised support for that carer in a timely way
• Support the carer to enable facilitation timely discharge of the cared for from hospital to own home or most appropriate care facility if needed
• Identifying rising carer burden to take action before carer breakdown occurs. This includes physical, mental, emotional and financial burden
• Identifying opportunities for a Structured Medication Review for older adults who are prescribed and using multiple medications / polypharmacy which can contribute to their physical frailty and cognition worries
• Identify those at risk of being isolated and lonely and facilitate support to minimise this
• Support carers with understanding the importance of contingency planning usingthe plan in FrEDA and Carers Check
• Identify and support to accessTalking Therapiesservices for common mental health issues such as anxiety & depression
• Supporting carers who may be frail or who have physical and mental health conditions to remain well to continue caring
• Encourage at older adults with mental illness or concerns to seek help from OPCMHT.
• To educate carers on available services and opportunities to access social and therapeutic activities for carers supporting engagement to reduce isolation and loneliness
• Enable the carer to think about their wellbeing by accessing community support to manage caring responsibilities
• Ensure referral on to appropriate agencies to continue carer support once intensive support is complete
• Use local information to suggest social / therapeutic activities for carers, explaining local community resources and supporting engagement
• Support carers to effectively forward plan when necessary, updating FrEDA as needs and circumstances change, while seeking suitable supervision.
• Support monitoring and feedback on the project with Manager and wider FRAIL + Team to provide a picture of anticipatory care for future planning of need
• Support Carers to access preventative services that could enhance and/or maintain physical and mental wellbeing (eg. Healthy lifestyles, dental, optical care and reviews of medication)
•
• Participate in regular non-clinical Supervision (4-6 weekly) facilitated by the line manager and weekly / as needed group clinical supervision with the CIS South East Team
• Act with awareness and understanding of own limitation and professional boundaries
• Participate in and contribute the carers circumstances to PCN MDT and INT meetings
