Crynodeb
• To provide specialist Occupational Therapy assessment, treatment & review of patients advanced palliative conditions or complex deteriorating neurology. In an unstable or deteriorating stage of their condition. Supporting patients & their families with their complex or rapidly changing health care needs, predominantly in their place of residence.
• To work as a Band 6 Occupational Therapist with the Community Specialist palliative care Team as a member of the wider integrated interdisciplinary team. Which includes Occupational Therapist, Physiotherapist & Therapy Technician.
• The post holder will provide a comprehensive range of evidence-based assessment & treatment to adults in Coventry with a specialist palliative need in patients own home or other community setting.
• To follow defined referral with criteria to Specialist Palliative care Team.
• To liaise closely with members of the team & other Community services to support patient’s effective therapy.
• To provide advice & guidance to others, patients, carers & staff working in health & social care
• To support professional development of the staff within other services as appropriate
• To support & contribute toward service development & meeting key performance indicators
• To assist in the quality & activity monitoring services.
• To adhere to the lone worker policy
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
• To be an integral part of the Community Specialist Palliative Care Team.
• To provide evidenced based specialist occupational therapy or physiotherapy assessments and appropriate occupational therapy or physiotherapy intervention to patients referred to the service
• To liaise closely with members of the interdisciplinary team and wider multidisciplinary team as required, facilitating effective caseload management
• To utilise specialist skill and knowledge to manage a palliative care and neurological caseload. To patients in an unstable or deteriorating phase of their condition ensuring the physical, psychological, behavioural, perceptual, cognitive, sensory and environmental needs of patients are addressed.
• To adopt a patient centred and sensitive approach to therapy, collaborating with patients (and/or carer(s) where appropriate) in all aspects of the therapy process (assessment, planning, intervention, evaluation, reporting).
• To have high level of knowledge of a variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls; understanding the impact of multi pathological rapidly changing presentation.
• To utilise a comprehensive range of evidenced based specialist assessments and treatment to patients in their own home or in a clinic environment
• To assist in the quality and activity monitoring systems, to support evaluation of effectiveness of the service

• Demonstrate highly developed communication and motivational skills to facilitate patients to manage their condition
• To Demonstrate the ability to deliver or discuss difficult diagnosis and prognosis with tact, empathy, managing distressed or aggressive patients or carers/relatives.

• To respect the dignity and preferences of patients and obtain consent to therapy, in some circumstances you would be working in the patients’ best interests where consent cannot be gained due to capacity.
• To provide and receive complex and sensitive information, verbally, written and/or electronically, to and from consultants, GP’s, patients, colleagues and other agencies.
• Attend and participate in interdisciplinary meetings and other relevant meetings both within CWPT and UHCW as required
• To have a broad knowledge of voluntary and independent organisations relevant to the specialist area and establish effective relationships with local services in order to enhance the delivery and transfer of care.
• Attend staff meeting as required
• Ensure all relevant information about the patient’s treatment or progress is communicated to other team members involved and other agencies as required
• Communicate with patients and relatives/carers the reasoning for assessments and interventions and establish full understanding and consent prior to delivery
• Employ and utilise formal and non-formal forms of assisted communication with patients or relatives/carers with communication impairments
• Communicate well with other team members ensuring accurate understanding of delegated therapy programmes to establish safe individual practice alone in the community
• Ensure relevant communication from the team is passed to Lead Therapists as required.
• To work Collaboratively with other key partnership organisations to ensure effective, safe delivery and transfer of care.
• To be consulted as a source of information by MDT members seeking Advice/information.
• To Keep relevant statistics and information as required by the department
