Crynodeb
The post holder will participate in the shared caseload of one of the Trusts multidisciplinary Community Mental Health Teams in Later Life Services. The post holder will undertake needs assessments and contribute to the risk assessment of service users referred to the Team, who may have either time-limited mental health disorders or severe and enduring conditions. They will act as a care co-ordinator for service users who require Occupational Therapy. They will develop and negotiate care plans with service users, families/carers and other professionals and agencies, ensuring access to the appropriate interventions and opportunities, with the aim of promoting social inclusion and recovery. The post holder will carry out evidence based occupational assessments and write up reports of a high standard, which highlight how biological sociological and psychological factors interact to create obstacles to the satisfactory participation of an individual service user in valued social roles. From the case formulation the post holder will make recommendations to and access support from appropriate professionals and agencies, or deliver Occupational Therapy interventions with the aim of gaining or restoring a balance of meaningful roles for the individual. Where cases are complex the post holder will accept support from their clinical supervisor, and other senior Trust OT staff in the analysis and case formulation for the service user.
