Crynodeb
Lead complex advance care planning discussions with care home residents and families. Undertake annual holistic healthcare reviews in line with EHCH requirements. Act as an independent prescriber, responsible for medication optimisation, deprescribing and prescribing anticipatory Just in Case (JIC) medications. Complete and update ReSPECT forms, Treatment Escalation Plans (TEPs) and ADRTs. Support proactive care planning to reduce avoidable hospital admissions. Work collaboratively with GPs, care coordinators and the wider MDT to maintain continuity of care. Document assessments and plans accurately using EMIS templates, including frailty scoring and dementia care planning where appropriate.
