Our health partners are creating a community intervention offer jointly with us. This means that we have the chance to develop within a multi-disciplinary context, share information and together deliver better outcomes. Opportunities exist around the new pathways supporting people to be safely discharged from hospital. Working with and across our primary care networks, we are finding better options to provide the wrap around care that helps people to stay safe and supported at home and keep them in their community at end of life.
Cornwall has a Net Zero carbon strategy to reflect our priories and responses to the climate emergency. We have all the challenges associated with being a destination for holidays and retirement and also significant contrasts between the very well off and communities that rely on the local food bank.
