CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

To Work within and across professional and organisational boundaries, liaising with GP’s, other professionals, and voluntary agencies to deliver individualised and expert clinical care to a high standard.

To support the locality manager/clinical lead and colleagues in delivering high quality care that delivers best value and meets the requirement of commissioners.

To take responsibility for managing members of the team, including undertaking appraisals with the support of senior staff.

To provide skilled, effective and evidence based clinical care to patients in a variety of community settings.

To use enhanced skills and knowledge to assess physical and psycho-social needs and develop a comprehensive and concordant plan of care with patients, carers and with wide MDT as needed.

To make risk assessments, establishing the safety or otherwise of patients’ ability to stay at home, or return home safely from another care setting, providing, and arranging necessary nursing and supportive packages.

Provide autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/or complex health needs and long-term conditions.

To support appropriate and timely case finding of patients suitable for the service including working to support in-reach to local acute trusts as needed.

To provide evidenced based clinical/therapeutic interventions based of ‘best practice’ in order to improve health outcomes and promote choice, e.g. Intravenous therapy (IV), Palliative care interventions, Catheter and bowel management, post operative anticoagulation administration and insulin management, prevention of admission assessments.
