SHARE is a 24/7 service that supports people who experience mental distress that impacts on their daily living. SHARE is a 12- bed social care provision that provides short term placements for periods of assessment under the Care Act 2014.
Two crisis beds are available to support individuals requiring crisis intervention as alternative to hospital admission.
One respite bed which provides short stays away from the home environment enabling carers to have periods of independence in their lives.
The service supports people from age 18, with no upper age limit
Referrals can be taken from professionals in Adult Social Care, Community Mental Health Services and Acute/Rehab Wards or internally from people stepping down from crisis beds for a period of assessment or intervention.
The aim of the service is to support people on the recovery pathway, so they lead more independent lives in the community, safely and to the best of their abilities.
The postholder will work with individuals, assessing their needs, co-producing goal orientated support plans. The postholder will work in conjunction with Adult Mental Social Workers and Clinicians from Community Mental Health Services,
We are seeking an enthusiastic and committed colleague to join our friendly team, the successful applicants will be expected to be imaginative and resourceful. They will have a positive and optimistic view of the process of change. Applicants should understand the issues surrounding and supporting the recovery of people who experience a serious mental illness.
Applications are invited from people with experience of mental health and social care settings, who can demonstrate their ability to work as a team member without direct supervision. They will be expected to hold a recognised certificate in Health and Social Care level 3 or demonstrate experience within residential mental health settings with a willingness to commit and undertake self-led study to complete qualification within set time frames.
We are committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expect all staff to share this commitment. Applicants to posts that are exempt from the Rehabilitation of Offenders Act will require a DBS from the Disclosure and Barring Service before the appointment is confirmed.
