Crynodeb
To monitor and evaluate care programmes for patients and carers, promoting effective co-ordination and continuity of patient care. To communicate formally and informally with the DN team leader to ensure best practice and consistency with the community and intermediate care service in TSDFT To be responsible for the performance of risk assessments for patients, staff and carers, in various working environments and act on the results. To utilise teaching, assessments and mentoring skills with students and team members. To provide an effective learning environment. To identify opportunities for professional development of team members through the development review and appraisal process. An ability to provide and receive complex, sensitive and contentious information: using empathy and reassurance skills To liaise with patients/relatives who could be unpredictable/or challenging in their responses because of stress related problems To undertake Specialist Nursing and Continuing Care assessments (with appropriate training) Please refer to your suitability to the post in your supporting information from the role requirements or person specification.
