Summary

Job Purpose To providehigh-quality, autonomous clinical care across the Primary Care Network (PCN), delivering safe, effective assessment, diagnosis, treatment and management of patients with undifferentiated and complex conditions. Key responsibilities Clinical Care Independently assess, diagnose, plan, implement and evaluate care for patients presenting with undifferentiated and complex conditions. Undertake comprehensive history taking and physical examinations. Analyse clinical findings and investigation results to develop differential diagnoses. Initiate appropriate treatment plans, referrals or admissions as required. Prescribe medication as an Independent Non-Medical Prescriber in accordance with national and local guidelines. Conduct home visits, care home reviews and ward rounds as required. Provide same-day and urgent clinical assessment where appropriate. Chronic Disease & Complex Care Manage patients with long-term conditions and multimorbidity. Undertake structured medication reviews in collaboration with pharmacists and GPs. Support anticipatory care planning and end-of-life discussions where appropriate. Develop and review personalised care plans. Enhanced Health in Care Homes (EHCH) Lead and participate in regular care home ward rounds. Provide proactive, coordinated care for care home residents. Identify deterioration early and implement management plans to reduce avoidable admissions. Work collaboratively with care home staff, community services and GPs. Ensure accurate and timely documentation and coding of care delivered. Frailty & Anticipatory Care Identify and manage patients within our early adopter frailty programme, using appropriate assessment tools and clinical judgment. Undertake comprehensive geriatric assessments where appropriate. Develop and implement personalised care plans for frailty patients, focusing on improving quality of life and reducing hospital admissions. Work collaboratively with the PCNs multidisciplinary team to provide holistic care for frailty patients, including social prescribing and community support services. Leadership and Clinical Governance Act as a senior clinical decision-maker within the PCN. Provide clinical advice and support to nursing and allied health colleagues. Contribute to development and review of clinical pathways and protocols. Participate in multidisciplinary meetings and case discussions. Support quality improvement initiatives. Governance & Risk Management Practice in accordance with the NMC Code and Advanced Practice framework. Contribute to clinical audit, service evaluation and quality improvement cycles. Participate in incident reporting and significant event analysis. Ensure compliance with safeguarding (Adults & Children Level 3), infection prevention and medicines governance standards. Maintain professional registration and revalidation requirements. Engage in peer review and receive appropriate clinical supervision. Service Development & Continuous Improvement Contribute to service development initiatives aligned to PCN priorities. Support implementation of new clinical pathways and digital tools, designed to improve patient flow and care coordination. Provide clinical input into evaluation of new systems or service models introduced within the PCN. Education and Workforce Development Provide mentorship and clinical supervision to junior staff and ARRS roles where appropriate. Support training and professional development within the MDT. Maintain own continuing professional development (CPD). Promote evidence-based practice across the PCN. Research, Audit & Data Quality Participate in clinical audits and evaluation of service effectiveness. Support accurate clinical coding (including SNOMED) to ensure data integrity. Contribute to performance monitoring and reporting requirements. Use available data to support improvements in patient care. Administrative & Operational Responsibilities Maintain accurate, contemporaneous and legally compliant clinical records. Work across PCN practices and care homes as required. Ensure compliance with data protection and confidentiality policies. Participate in service planning discussions as required. Professional Accountability Accountable for own professional practice. Maintain valid NMC registration. Maintain Independent Prescriber status. Work within defined scope of practice and seek advice when appropriate. Maintain professional indemnity cover. Hold enhanced DBS clearance. Wider responsibilities In addition to the primary responsibilities, the PCN Advanced Clinical Practitioner has the following wider responsibilities: a. Support the delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives b. Lead on the delivery of training, mentoring and guidance to other clinicians and staff on medical emergencies c. Support virtual and remote models of consultation and support, including e-consultations d. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner Other Information The post holder must be committed to safeguarding and promoting the welfare of vulnerable adults. The role will require travel between care homes within the PCN area, therefore a valid driving licence and access to a vehicle are essential.
