Summary
Lampton Frailty Unit is a 29-bedded general medical unit. It is supported by a dedicated team of therapists, who work closely with the nursing and the medical team. Many of the patients have complex needs and will require safe and careful discharge planning.
An exciting opportunity has become available on the newly opened Lampton Frailty Unit (LFU) for a Band 3 Ward Flow Coordinator. This is a full time post, 37.5 hours per week Monday –Friday.
We are looking for a friendly, reliable and motivated individual to join our hardworking team. The role involves supporting the whole unit coordinating clinical and non-clinical activities to ensure smooth and well organized flow within and outside the unit.

· To support efficient and seamless ward processes from admission, transfer and discharge.
· To support the Ward Manager and ‘Nurse in Charge’ by facilitating board-rounds and being responsible for monitoring all actions required, ensuring they are done in a timely manner to provide a seamless service for patients’ through the admission, transfer and discharge process.
· To be responsible for the data input into clinical systems that support flow and timely discharge of patients.
· To assist with all discharges and support the MDT team by using effective coordination and communication skills to enhance the discharge process; including completion of Early Discharge Notification (EDN), electronic Need Based Assessment (eNBA) supervised by a Registered Practitioner. In supporting the flow of the ward, the post will entail non-clinical tasks and administrative tasks under the supervision of the Ward Manager and Nurse in Charge.

· Assist members of the team in the coordination of the clinical activities
· Use clinical standards to participate in the process of monitoring the quality of care delivered.

Clerical Responsibilities
· To receive patients, relatives and visitors on the ward in a friendly and welcoming manner, referring to nursing staff as necessary.
· To deal with any telephone enquiries and ensure messages are recorded or passed on as appropriate.
· To undertake clerical tasks relating to the safe and efficient running of the ward. This will include:
o The CERNER system is kept up to date.
o All new patients are screened on arrival for care needs.
o Discharge planning to begin from the start of the patient’s journey.
o Review case notes to check if there is an existing care package.
o Liaise with the nurses with regards to the care needs of the patient.
o Assist completion of discharge notifications and needs based assessments.
o Follow up discharge summaries and medications to take home.
o Arrange patient transport.
o Liaise with discharge lounge to facilitate early discharges from the ward.
o Check and ensure timely dispatch of electronic discharge summaries.
o Support with fast track discharges
o Speak with family/ carers to organise private care packages/ carers/ cleaning/ shopping support etc as needed by the patient
· To order and check notes and stationaries, to maintain adequate stock levels as directed by the ward manager or Nurse in Charge (NIC).
Communication
· Ensure all communication, which may be complex, contentious or sensitive, is undertaken in a responsive and inclusive manner, focusing on improvement and ways to move forward.
· Ensure all communication is presented appropriately to the different recipients, according to levels of understanding, type of communication being imparted and possible barriers such as language, culture, understanding or physical or mental health conditions.
· To accord respect quality, dignity and confidentiality to each patient, relative and visitor.
