Summary
Provide holistic care for Adults with a Learning Disability and complex needs requiring specific health interventions. To be responsible for a caseload of service users who require close monitoring under the supervision of a Band 5 Community nurse. Be allocated to work with service users and assist in the development of care plans and risk assessments along side the clinical team including undertaking observations and monitoring. Provide a rich and stimulating environment enhancing the lifestyle of the service users and their individuality
