Crynodeb
Support patients to get the best from their medicines, including switching medications in line with agreed protocols. Improve repeat prescribing processes within General Practice, promoting repeat dispensing and online ordering. Minimise clinical risk and reduce medicines waste. Work as part of a multidisciplinary team in a patient-facing role, clinically assessing and treating patients. Take direct responsibility for the care and prescribing of defined patient cohorts. Deliver and develop pharmacy-led medicines optimisation services. Ensure practices remain up to date with national guidance and evidence-based practice. Lead and promote medicines optimisation within a specialist area, ensuring best value, reduced harm and improved outcomes. Lead evaluation and audit of service delivery to maintain and improve quality. Work in accordance with Medicines Optimisation principles and up-to-date national guidelines.
