This is a great opportunity to build your clinical and leadership skills while supporting people with complex health needs in the community.

You’ll work as part of an experienced community nursing team to provide personalised care for people living with long‑term conditions. Your work will help people stay well at home, avoid unnecessary hospital admissions, and feel confident in managing their health. Working closely with patients, carers and partners across health and social care, you’ll play an important role in shaping safe, coordinated, and responsive care.



What you’ll do:
• Complete holistic assessments and create care plans that meet individual needs
• Act as a care coordinator, supporting smooth transitions and effective communication
• Deliver a range of clinical interventions such as wound care, IV therapies and end‑of‑life support
• Monitor caseloads proactively and respond to changes in patients’ conditions
• Maintain accurate, up‑to‑date electronic care records
What we’re looking for:
• A registered nurse with up‑to‑date clinical knowledge and evidence of ongoing development
• Experience of community nursing or caring for people with long‑term conditions
• Strong communication skills and confidence working with patients, families and partners
• Ability to work independently, prioritise work and support colleagues through supervision
• Willingness to work towards non‑medical prescribing if not already achieved
