Summary

Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the lead Pharmacy Technician Assist with medication safety monitoring systems e.g high-risk drugs Liaise with clinicians and administrative staff to resolve medicine- related queries. Support medicine reconciliation of patients whose care is transferred back into primary care in a timely and effective manner liaising with patients and other providers to ensure patients receive appropriate medication post discharge Carry out medicines optimisation tasks including effective medicine administration, supporting medication reviews and medicines reconciliation of transfer of care Care Quality Commission-Work with the practice to ensure compliance with CQC standards where medicines are involved Implement changes to medication that result from CAS-MHRA alerts, product withdrawal and other local/ national guidance Training and Development Take responsibility for own development with relevant evidence based knowledge and competence in all aspects of the role to meet clinical governance guidelines for Continuing Practice Development Work to deliver the NHS contract requirements related to the Practice (including the terms of Quality and Outcomes Framework) Governance Produce complete and accurate records of patient consultation in line with best practices, confidentiality, policies and procedures Deliver care according to NHS guidance, NICE guidelines and evidence-based care Take part in maintaining quality governance systems and processes across the Practice and its activities. Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them Ensure compliance with policies, procedures and guidelines for self and others by taking action or alerting the senior management team if the Practice appears to contravene policy or if there are concerns over any aspect of patient care. Confidentiality Maintain confidentiality of information, acting within the Data Protection Act's terms and Caldicott's guidance on patient confidentiality. Maintain an awareness of the Freedom of Information Act. Information relating to patients, carers, colleagues, healthcare workers and the practice business is restricted to authorised persons following the Practice policies and procedures regarding confidentiality and the protection of personal and sensitive data. Health and Safety Every employee should take reasonable care for the Health and Safety of themselves and others, including using protective clothing and cooperation with the practice in meeting its responsibilities under the Health and Safety at Work legislation. In addition, the post holder must be aware of individual responsibilities under the Health and Safety at Work Act and identify and report, as necessary, any untoward accident, incident or potentially hazardous environment. Codes of Conduct Comply with the Code of Professional Conduct set by the General Pharmaceutical Council.
