We are seeking an experienced and motivated Band 7 Macmillan Dementia and Cognitive Frailty Practitioner to join Dementia Team within our newly established Supportive Oncology Directorate at The Christie. This senior clinical role offers the opportunity to lead, influence and deliver high-quality, person-centred dementia care within a specialist oncology setting.

You will work autonomously while supporting the Dementia and Cognitive Frailty Lead to develop and embed best practice across the Trust. Acting as a visible clinical leader and champion for dementia care, you will improve patient and carer experience, support staff, and ensure safe, equitable pathways for people living with dementia or cognitive frailty undergoing cancer treatment.

This role is ideal for a clinician with advanced assessment skills, strong leadership capability and a passion for service development, education and improving outcomes for some of our most vulnerable patients.

You will undertake specialist holistic assessments for patients with dementia or cognitive frailty, planning, delivering and evaluating care in line with evidence-based practice. You will manage a complex caseload autonomously, providing expert advice and guidance to multidisciplinary teams across inpatient and outpatient areas.

The post holder will provide clinical leadership for dementia care within the Trust, supporting implementation of pathways. You will contribute to MDTs, discharge planning and best-interest decision-making, while working closely with community, primary care and mental health services.

You will lead and deliver dementia and delirium education across the organisation, support service development, contribute to audit and research activity, and promote continuous improvement in patient experience, safety and quality of care.

The Christie is one of Europe’s leading cancer centres, treating over 60,000 patients a year. We are based in Manchester and serve a population of 3.2 million across Greater Manchester & Cheshire, but as a national specialist around 15% patients are referred to us from other parts of the country.

We provide radiotherapy through one of the largest radiotherapy departments in the world; chemotherapy on site and through 14 other hospitals; highly specialist surgery for complex and rare cancer; and a wide range of support and diagnostic services. We are also an international leader in research, with world first breakthroughs for over 100 years.

We run one of the largest early clinical trial units in Europe with over 300 trials every year. Cancer research in Manchester, most of which is undertaken on the Christie site, has been officially ranked the best in the UK.

DUTIES AND RESPONSIBILITIES

Clinical

· Perform specialist holistic assessments when assessing the needs of the person with dementia or cognitive frailty when receiving care for oncology treatment within the Christie NHS Foundation Trust.

· Plan, implement and evaluate care delivery according to changing health care needs to ensure continuity of effective patient care.

· Perform advanced clinical skills in assessment and diagnosis of dementia and management of associated psychological and physical health needs.

· Demonstrate ability to work autonomously and manage own caseload.

· Provide expert professional advice to multi-disciplinary colleagues regarding the care of patients with dementia and their associated needs.

· Provide guidance to medical colleagues on how to identify and prevent reversible causes of confusion.

· Provide a clear focus for clinical leadership, providing a highly visible and authoritative presence, responsible for ensuring the delivery of high-quality dementia care, ensuring that patient needs are placed at the centre of care delivery.

· Collect, collate, evaluate and report information, maintaining accurate patient records in line with clinical governance and risk-management.

· Involve patients and carers/relatives in the planning and delivery of care and ensure this practice is routine within the trust. Facilitate the development of services to assess and support carer needs and enhance the patient/carer experience.

· Attend MDTs and discharge planning meetings of patients identified with dementia wherever possible.

· Work collaboratively with other professionals and agencies to ensure patient needs are met both within the trust and in the community. Ensure access and liaise with services in the community which can address ongoing needs, e.g. intermediate care and re enablement teams.

· Link with primary care, local memory clinics and community mental health teams as appropriate.

· Assess plan and implement programmes of evidenced based care for client groups.

· Ensure interventions are undertaken in line with best practice guidance.

· Act as a resource and provide specialist advice, information and support to Trust staff.

· Identify local, demographic, cultural and environmental factors that influence the health and quality of life for client group.

· Provide a visible clinical profile, having both an advisory and clinical input into patient care.

· Support in the preparation of accurate and concise reports both verbally and written.

· Use advanced communication skills in a variety of settings.



Clinical Leadership

· Ensure that ‘This is Me’ forms are available and utilised within the Trust for adults living with dementia.

· Chair, deputise or participate on dementia working groups as required, or act as a clinical advisor in relation to dementia on other trust-wide working groups.

· Actively engage with Trust staff to assist and empower them to provide safe and equitable care for adults living with dementia.

· Ensure that the Confusion Care Pathway is utilised within the Trust.

· Ensure the principles of the Mental Capacity Act are understood and applied for adults living with dementia accessing Trust services.

· Work with Trust staff in ensuring a smooth and safe pathway for adults living with dementia whilst in the oncology setting.

· Provide a point of contact for advice and support for adults living with dementia and their relatives in relation to their care needs whilst in the oncology setting.

· Accurately and promptly maintain a database of dementia and delirium activity to be recorded and reported.

· Provide and deliver Dementia and Delirium training at all levels for all multi-disciplinary staff within the Trust.

· Ensure the views of adults and their family or carer are captured through the patient experience surveys.

· Attend Best Interest Meetings and MDT meetings when required.

· Produce written reports as requested by Line Manager.

· Deputise in order to present or attend local, national meetings or conferences in order to develop or share practice



Education and Development

· Provide specialist, expert clinical knowledge to clinical colleagues, patients and carers/relatives both within and outside the Trust.

· Identify learning needs, help plan, implement and evaluate programmes of education to meet identified need.

· Assist in developing and implementing a programme of education on Dementia Management for health care professionals in the Trust. Monitor the effectiveness of the education programme in improving staff knowledge and competence in Dementia care, and improving the standard of person centred dementia care in the hospital.

· Educate and support staff in the specialist oncology hospital on how to manage in-patients with dementia in terms of delirium prevention, reorientation, communication, managing distressed behaviours and delivering person centred care.

· Develop pathways of care for inpatients with dementia in order to optimise standard of care received during their inpatient journey, in line with National Guidance e.g. The National Dementia Strategy, CQUINs, The National Dementia Audit, NICE guidelines for Dementia.

· Develop pathways for discharge planning, working in collaboration with discharge planning services to ensure timely discharge of patients with dementia and delirium and delivery of an appropriate package of community care on discharge.

· Assist with developing a programme to monitor and improve the ward environment for patients with dementia e.g. provision of meaningful activity, orientation aides, and clear signs. Identify and intervene where circumstances constitute an unsafe clinical environment, reporting any such instances to the divisional nursing lead.

· Liaise with other professionals involved in improving dementia care e.g. dietetics, palliative care, to ensure relevant guidelines and protocols in nutrition assessment and pain management are implemented appropriately in patients with dementia.

· Ensure own compliance and compliance of others with regards to mandatory training and professional requirements.

· Address specific health targets related to own area of practice, e.g. NICE, Improving Outcomes Guidance and Department of Health Guidelines.

· Receive clinical supervision as part of own professional development.

· Maintain up to date skills and knowledge and maintain awareness of professional issues.

· Maintain a professional portfolio.

Research & Audit

· Participate in the evaluation of clinical practice through audit of service, supporting to develop and manage strategies to address any shortfalls.

· Demonstrate knowledge of current research in all aspects of work and to advise others on the implementation of relevant research findings

· Participate in research projects in response to identified needs

· Disseminate results/research findings via local team and/or national/international publications.

· Support and promote the shared objectives of the multidisciplinary team by working closely to ensure that best practice is achieved.
