We are looking for a motivated and compassionateBand 5 Community Nurseto join our dynamic Community Nursing Team. This is an exciting opportunity to deliver high‑quality, person‑centred care to patients in their own homes, helping them to maintain independence, manage long‑term conditions, and prevent unnecessary hospital admission.
As a key member of the multidisciplinary team, you will provide skilled clinical care, build meaningful therapeutic relationships, and contribute to the ongoing development of community services.
Main Responsibilities
• Deliver safe, effective nursing care to patients in their home environment.
• Undertake holistic assessments, develop personalised care plans, and evaluate outcomes.
• Perform a wide range of clinical interventions including wound care, venepuncture, catheter care, medication administration, and management of long‑term conditions.
• Work collaboratively with GPs, allied health professionals, social care, and voluntary sector partners.
• Prioritise and manage your own caseload with support from senior colleagues.
• Promote self-management, independence, and patient empowerment.
• Maintain accurate, timely clinical documentation in line with professional standards.
• Participate in service development, quality improvement, and team learning activities.
About You
We are looking for someone who is:
• A registered nurse (NMC) with evidence of ongoing professional development.
• Passionate about community-based care and improving patient outcomes.
• Confident working independently and managing your own schedule.
• Able to problem-solve, adapt, and remain calm under pressure.
• An excellent communicator with strong interpersonal skills.
• Committed to delivering compassionate, person‑centred care.
• A team player with a positive, proactive approach.
A full UK driving licence and access to a vehicle for work purposes is essential.

To work as a member of a neighbourhood nursing team that delivers person centred integrated care in the community. The aim of the service is to support people to remain as independent at home as long as possible, proactively facilitating self-care.
Care is provided in line with Buurtzorg principles including ‘first coffee then care’; spending time getting to know patients and their families enabling them to make the right choices and decisions together. Care is focussed on improving self-reliance and empowerment; seeing patients as a whole and supporting as required their health, social and psychological needs.
To work as an integral part of a team of nursing staff providing consistent care to a defined community caseload of patients 365 days a year, 24/7 hours a day. This requires flexibility and commitment to maintain effective service provision for the team’s caseload.
The role requires close collaboration with the GP and other health, social care and voluntary organisations including but not limited to therapists, mental health, social workers, care navigators, case managers etc.
Your role will support the team to achieve the following core aims:
1. Having happy patients: through direct access, continuity of care, flexibility of care provision and providing quality clinical care.
1. Having happy nurses and caregivers; by working collaboratively and flexibly with your team colleagues and being empowered to make decisions as a team to best meet the requirements of your patients and their families.
You must have a UK driving licence and access to a vehicle to use for work purposes.
