Title: Practice Nurse Responsible to: Nurse Manager Hours of work: 2-3 days Job summary The Practice Nurse will be responsible for delivering general practice nursing services to the entire patient population, with a focus on evidence-based care for long-term conditions. The role includes both managing chronic diseases and providing preventative health care. The nurse will work autonomously, using critical thinking and clinical decision-making skills to manage patients effectively. Collaboration with the wider healthcare team will be essential to meet the practice's needs and contribute to policy delivery. The ideal candidate will have a strong interest in respiratory care (Asthma, COPD, Bronchiectasis, Spirometry), with the potential to take on a leadership role in chronic disease management for respiratory patients following appropriate training. Key responsibilities Clinical Practice Provide safe and effective clinical practice, adhering to boundaries of competence and seeking support as needed. Assess, plan, implement, and evaluate treatment programs, focusing on patient well-being and the management of long-term conditions. Work collaboratively with healthcare professionals and refer when necessary to manage chronic conditions using both pharmacological and non-pharmacological methods, adhering to evidence-based protocols. Regularly review medications for therapeutic effectiveness in line with patient needs and national practice guidelines. Support patients in adhering to prescribed treatments and lifestyle modifications. Deliver health promotion and self-care advice to encourage patients to take responsibility for their health. Administer vaccinations and immunisations under national guidelines, including for both adults and children. Provide support for patients traveling abroad, including risk assessments, health promotion, and vaccination under PGDs. Provide acute and chronic wound care, assist in minor surgical and family planning procedures, and manage prescribed medications. Ensure strict infection control and patient safety practices are maintained.
