Summary
Undertake comprehensive patient assessments, including history taking, physical examination, diagnosis, and management of acute, chronic, and complex conditions; Independently manage own caseload, including same-day urgent appointments; Prescribe medications independently (where qualified) in line with practice policy and national guidance; Request, interpret, and act upon diagnostic investigations (e.g. blood tests, ECGs, imaging); Make timely referrals to secondary care, community services, and multidisciplinary teams; Manage long-term conditions (e.g. diabetes, asthma, COPD, hypertension); Provide health promotion, disease prevention, and patient education; Work collaboratively with GPs, nurses and pharmacists; Contribute to service development and quality improvement initiatives; Maintain advanced clinical competence and professional registration; Participate in clinical supervision, appraisal, and revalidation; Support the learning and development of students and junior staff as appropriate; Engage in continuing professional development (CPD) in line with NMC and practice requirements; Work within NMC Code of Professional Conduct and scope of practice; Ensure accurate, timely, and confidential clinical documentation using electronic patient records; Participate in clinical audit, significant event analysis, and quality improvement activities; Adhere to safeguarding policies and identify vulnerable patients; Maintain awareness of current NHS, CQC, and local commissioning requirements.
