You will provide operational management support to the Homeless Health team and will work closely with the GP Clinical Lead in the evaluation and ongoing development of the service. In addition to the three years funding to continue the development of our inpatient service, additional money has been provided for one year to more effectively in-reach into Emergency Department. You will oversee the development of this service.
The Homeless Health Team works in close partnership with the wards to identify people experiencing homelessness whilst they are in hospital, build therapeutic relationships with them, assist the ward multidisciplinary team to maximise the benefit of these admissions, and then support these vulnerable patients during and after discharge – ensuring that they are fully linked in to all the health care and support services they need when the team finally discharges them.
