Summary


An opportunity has arisen for a Clinical Fellow to join the Cardiac Surgery department in the Trent Cardiac Centre on the City campus of Nottingham University Hospitals NHS Trust. Applications are invited for a senior cardiac surgical fellow aimed at allowing successful candidates to continue to develop their surgical skills and to be involved in the provision of adult cardiac surgery critical care within the Trent Cardiac Centre (Nottingham University Hospital City Campus), this will be a permanent position. The post will be of value to senior doctors with experience in cardiothoracic surgery (more than 5 years) and post-FRCS exit exam. There will be an assigned Educational and Clinical Supervisor, formulation of learning agreements and regular formal assessments of progress based on the ISCP Cardiothoracic curriculum. Candidates must be able to demonstrate experience in the assessment and management of the critically ill patient. There are currently 5 full time consultant cardiac anaesthetists, all of whom are skilled in TOE use and who will provide training in theatre and on CICU in this regard if required. The 5 Consultant cardiac surgeons will provide training in the surgical aspects of the role tailored to the level and need of the appointee.

The post will involve patients’ management on the Cardiac Critical care unit and the fellows are responsible for everyday care of the patient including daily ward rounds in CICU and the ward. They are expected to attend weekly teaching sessions, cardiology/cardiac surgery MDT and echocardiography meetings. An appointee will have opportunities to attend outpatient clinics, taking part in the pre-operative assessment clinics and also in post-operative clinics. The post-holder will participate in the cardiac surgery SpR on-call rota (1:6). The post described is not officially a designated training post and does not carry an NTN number.

When on call, the successful candidate will provide CICU cover (covering the nurse practitioners) and senior advice to the post-operative cardiac surgery ward based patients (junior cover of ward patients being provided by the Hospital at Night scheme). The successful candidate will be expected to contribute to the research and audit projects ongoing within the department, and will participate in undergraduate and postgraduate teaching programmes. The successful candidate will be encouraged to attend courses and meetings for continuous professional education. There is plenty of opportunity for attending and presenting in MDTs and in educational and journal club.

The inpatient cardiac surgery service at Nottingham University Hospitals takes place in the recently opened Trent Cardiac Centre located on the City Hospital campus. It is based around a 16-bed cardiac critical care unit and one 20-bed ward. There are two cardiac catheter laboratories and two cardiac theatres. Thoracic surgery patients have access to the general intensive care and hospital high dependency unit and are managed separately. All cardiac radiographic images are stored digitally and are available for review via a dedicated computer network. Intensive care management is provided jointly by surgeons and anaesthetists, however the anaesthetic team take responsibility for leading a daily ward round and the work is supported by 6 specially trained nurse practitioners, who provide first line management of patients.



The appointee will spend a significant amount of their time in theatre developing surgical skills. The remainder of their time will involve patient’s management on the Cardiac Critical care unit and the fellows are responsible for everyday care of the patient including daily ward rounds in CICU and the ward. They are expected to attend weekly teaching sessions, cardiology/cardiac surgery MDT and echocardiography meetings. An appointee will have opportunities to attend outpatient clinics, taking part in the pre-operative assessment clinics and also in post-operative clinics.



The post-holder will participate in the cardiac surgery SpR on-call rota (1:6). This is an on-call non-resident system. The post described is not officially a designated training post and does not carry an NTN number. When on call, the successful candidate will provide CICU cover (covering the nurse practitioners) and senior advice to the post-operative cardiac surgery ward based patients (junior cover of ward patients being provided by the Hospital at Night scheme). The candidate must be responsible for up to date data input, typing / dictating the letters and discharge summaries in a timely manner.
