The role of Admission Prevention and Pathway Manager will be based within the Emergency Village (EV) – comprising of the Emergency Department (ED), and Emergency Assessment Unit (EAU), and the Control Centre.

The successful canddiate will provide clinical support, assistance and professional expertise to all patients which they and the EV team identify as being able to return home but may require more support.

The role’s purpose is to focus on early identification of patients who require the most support for discharge at the point of admission, or those who may need a short admission on a ward and have already been identified in the EV. Our ethos is to take a ‘Strengths Based’ approach to work with the patient to identify what is working well for them, and what else they may need to return home, and maintain their health and social conditions with support from Housing, the Voluntary Sector, Primary Care, Urgent Response Teams, and local support networks.



Professional Duties

-Provide clinical expertise and support in the assessment and management of people attending ED and EAU to prevent unnecessary admission to Acute Care and to facilitate collaborative MDT management for people closer to home.

-Ensure a discharge plan is considered and established on attendance to the EV.

-Establish and maintain effective communication with individuals and groups about difficult or complex matters, overcoming any problems in
communication.

-Assess for and develop individualised treatment programmes to prevent unnecessary admission to hospital, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required.

-To be professionally legally responsible and accountable for aspects
of own-clinical work.

-To be confident and coherent in sound clinical reasoning to ensure risk mitigation for discharge plans.

-Employ the Making Every Contact Count Model to advise and offer
support on how to improve and maintain health and wellbeing.

-Use a ‘Strengths Based’ approach to work collaboratively with
patients and service users promoting their opportunity to co-produce their plan of care, recognising and utilising their individual
strengths and abilities.

-Complete the Continuing Health Care Screening Tool, planning and processes.

-Refer to Home First colleagues for support when required.
