Primary key responsibilities: The post holder will be required to: Coordinate CAD sessions: Work closely with Social Prescribing Managers and the Operational Manager to plan and deliver CAD sessions Support the development of and adhere to Standard Operating Procedures (SOPs) Develop patient pathways and referral routes Manage an inbox and respond to queries and appointment requests, booking appointments where required Deliver coordination of the service, including rotas, documentation, equipment and locations, supported by service managers Build relationships and networks with key stakeholders and community partners Support data collection, evaluation and impact analysis Produce reports and provide updates on progress of the project within the budget Embed learning from the service Attend meetings on behalf of the service Deliver Social Prescribing Support and Intervention: Provide person-centred support based on the persons priorities, interests, values and motivations Engage positively with the aims and values of the Working Well initiative, promoting health, independence and employability Work closely with the Occupational Therapist and Social Prescriber to ensure an integrated, and holistic approach to patient care Work autonomously within agreed service parameters Contribute to MDT discussions and joint decision-making to support positive employment and health outcomes. Use the clinical system to maintain accurate and high-quality patient records, in line with data protection principles Support community groups and VCSE organisations to receive referrals Identify and escalate Safeguarding concerns as appropriate to both Adult Social Service sand NGPS line management. Secondary responsibilities: In addition to the primary responsibilities, the Social Prescriber may be requested to Promote social prescribing, its role in self-management, and the wider determinants of health across all stakeholders and partners Attend meetings or events on behalf of NGPS and the Social Prescribing service Use evidence and data to contribute to service improvement Participate in audits as directed by the audit lead Manage sickness and holiday absence in line with your patient calendar and referral stream. Keep practices informed of any absence as well as NGPS line management with as much advance notice as possible for the smooth running of the service Communicate effectively with internal and external stakeholders Core principles Treating others with empathy and kindness. Patients entrust and permit us to collect and retain sensitive information relating to their health and other matters pertaining to their care. They do so in confidence and have a right to expect all staff will respect their privacy and maintain confidentiality. It is essential that, if the legal requirements are to be met and the trust of our patients is to be retained, all staff must protect patient information and provide a confidential service. Collaborative Working All staff are to recognise the significance of collaborative working and understand their own role and scope and identify how this may develop over time. Staff are to prioritise their own workload and ensure effective time-management strategies are embedded within the culture of the team. Teamwork is essential in multidisciplinary environments, and the post holder is to work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working and work effectively with others to clearly define values, direction and policies impacting upon care delivery. Effective communication is essential, and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner. All staff should delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence. Plans and outcomes by which to measure success should be agreed. Managing Information All staff should use technology and appropriate software as an aid to management in the planning, implementation and monitoring of care and presenting and communicating information. Data should be reviewed and processed using accurate SNOMED codes to ensure easy and accurate information retrieval for monitoring and audit processes. Data Protection The post holder must at all times respect the confidentiality of information in line with the requirements of the Data Protection Act and principles. This includes, if required to do so, obtain, process and/or use information held on a computer in a fair and lawful way, to hold data only for the specified registered purposes and to use or disclose data only to authorised persons or organisations as instructed. Service Delivery Staff will be given detailed information during the induction process regarding policy and procedure. The post holder must adhere to the information contained within the organisations policies and regional directives, ensuring protocols are always adhered to. Security The security of NGPS is the responsibility of all personnel. The post holder must ensure they always remain vigilant and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks with anyone and are to ensure that restricted areas remain effectively secured. Likewise, password controls are to be maintained and are not to be shared.
