Crynodeb
The Acute Liaison Nurse will act as a clinical expert and professional resource to the multi-professional team, patients with a learning disability, their families and carers across the Trust.
The post holder will ensure the delivery of high quality, clinically effective care, utilising research, and collaborating with colleagues in everyday practice and work in partnership with the MDT to enhance professional standards of care across the speciality.
• The post holder will plan and deliver care in accordance with the Trust’s committment to deliver high quality care .
• The post holder will work in collaboration with mainstream hospital services to enable open and easy access to healthcare services for people with learning disabilities.
• The post holder will work alongside health professionals and senior hospital staff to achieve a positive experience and clinically appropriate outcomes, for patients with a learning disability.
• The post holder will initiate and support person centred approaches to the delivery of care within healthcare services.

• The post holder will be a key player in the implementation of the hospital’s action plan for people with a learning disability, including attendance at the hospital Learning Disability Steering Group
• The post holder will advocate on behalf of individuals with a learning disability unable to access mainstream health services.
• The post holder will act as a resource for the acute hospital regarding the complex health needs of people with a learning disability.
• The post holder will support the acute patient area, where necessary, in the assessment, planning, implementation and evaluation of care plans for patients who have a learning disability and highly complex needs.
