We are working closely alongside with a local authority in Wrexham to assist with the appointment of a Support Worker on a 1.5-months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: £97.12 - £128.15 per day

Summary:

This role focuses on supporting vulnerable adults, including older people and those with physical or sensory impairments, to live independently within their own homes. Working within a 365-day service, the Support Worker delivers person-centered care through short-term reablement and intermediate interventions designed to maximize independence following illness, accidents, or hospital discharge. The role is regulated by the Regulation and Inspection of Social Care (Wales) Act 2016, requiring staff to promote positive outcomes while managing risks and challenging behaviors.

Responsibilities:

Assist with daily tasks including washing, dressing, toileting, bathing, and personal hygiene.
Prompt medication intake, manage catheter care according to guidance, and assist service users with mobility using specified equipment.
Support food and drink preparation and assist with feeding where necessary.
Respond to emergencies and mobile response requests, including assessing uninjured fallers and using lifting equipment safely
Execute individual care plans and health action plans as agreed by Social Services and Health Professionals.
Essentials:

Must be registered with Social Care Wales and comply with the Code of Professional Practice for Social Care.
Experience in providing high-quality, person-centered support in a domiciliary, reablement, or residential setting.

Experience or capability in managing challenging behavior and communication difficulties
