Summary

CoreResponsibilities Clinical Assessment & First Point of Contact Work alongside the wider multidisciplinary team as a firstcontact clinician for patients presenting with undiagnosed or undifferentiated conditions. Use advanced skills in historytaking, physical examination, critical thinking and clinical decisionmaking to reach a safe working diagnosis and create an appropriate management plan. Provide timely assessment, treatment and diagnosis for patients attending the Practice.Clinical Autonomy & DecisionMaking Make independent, accountable clinical decisions within scope of practice. Determine when patients can be safely treated and discharged or when referral to other services is necessary. Request diagnostic tests to support clinical judgement.Prescribing & Medicines Management Generate EPS or FP10 prescriptions within your professional boundaries and in line with local and national prescribing guidelines. Apply evidencebased practice to ensure safe, effective, and appropriate medication decisions.Professional Standards & Governance Maintain high standards of clinical practice in accordance with HCPC, Department of Health and College of Paramedics guidance. Contribute to strong working relationships across the Practice to promote collaborative, patientfocused care. Support the implementation of new clinical pathways, service improvements and practicewide initiatives.Quality, Safety & Service Improvement Contribute to the achievement of quality targets and the delivery of safe, effective patient care. Support the development of new services and improvements to current services. Assist with enhanced services obligations where needed. Take part in handling patient complaints when requested and support shared learning from incidents, near misses, and feedback.Training, Development & Governance Complete all mandatory training, induction modules and ongoing CPD requirements. Actively contribute to clinical governance, audits, and quality improvement activities. Engage in annual appraisals, regular reviews of objectives and continuous development discussions. Provide visible, positive clinical leadership that encourages professionalism, compassion and effective teamwork. Participate in service redesign and training activities to continually develop the role.Public Health & Operational Responsibilities Support public health initiatives - e.g., flu vaccination clinics through advice or direct care. Maintain a clean, safe and efficient working environment at all times Additional Responsibilities Support delivery of QOF, IIF targets, incentive schemes, QIPP initiatives and other quality or efficiency programmes. Undertake additional tasks appropriate to the role and level of responsibility, ensuring work is completed in an effective and timely manner. Duties may evolve over time to meet service needs but will remain consistent with the role profile and level of responsibility.
