AM: Acute Frailty Service in A&E + MDT meeting PM: Acute Frailty Service in A&E This structure runs Monday to Friday, with paid weekend rota cover included This post would suit an experienced resident doctor in medicine with an interest in Care of the Elderly and an aspiration to further their experience in the speciality before committing to a future career path. Responsible for the clerking and day to day care of patients identified by the Frailty Team in A&E. They will undertake independent clinical review of patients and develop individualised care plans with particular reference to geriatric syndromes. Undertake sensitive discussions with patients, families and carers around advanced care planning, DNAR discussions, admission avoidance planning, NG feeding/risk feeding, end of life discussions and explaining clinical management plans to patients and their families. Make appropriate referrals to other health and social care professionals. Work proactively and collaboratively to expedite discharges, working closely with social care teams including discharge coordinators, discharge hub teams, community teams, GPs and care homes. Participate in daily Multi-disciplinary Team meetings and work closely with Allied Health professionals. Participate in audits, quality improvement projects and Frailty service development. Contribute to the delivery and development of education and training pertaining to the Frailty work-stream. Attend the active postgraduate programme with weekly lectures, radiology, medical and pathology meetings and medical unit presentations.
