Job Title: Care Coordinator (Care Operations) Reporting To: Head of Care (Nursing) Location: Horwich, Bolton (with regional travel across the North West) Hours: 37.5 hours per week, Monday to Friday, 9:00 am to 5:30 pm with one hour unpaid lunch. Occasional early mornings, late evenings, or weekend meetings may be required. On-Call Requirement: One weekend per month on a rolling rota. Driving Requirement: Essential Job Purpose The Care Coordinator is responsible for the operational management and coordination of the Care Team, ensuring that all care packages are effectively mobilised, staffed, and delivered in line with agreed timeframes, organisational standards, and regulatory requirements. Working closely with the Head of Care (Nursing), Senior Carers, and Team Leaders, the post holder ensures operational readiness, workforce stability, training compliance, and high standards of quality assurance across all services. Key Responsibilities Operational Delivery Coordinate the mobilisation and delivery of all commissioned care packages within defined timelines. Ensure all packages are appropriately staffed, scheduled, and operationally prepared prior to commencement. Monitor ongoing service delivery to maintain continuity and quality. Referral and Onboarding Management Manage the operational aspects of onboarding new care packages, including documentation, staff allocation, and communication with families. Coordinate safe and compliant offboarding of care packages when required. Rota and Workforce Management Develop and maintain daily and weekly rotas to achieve high levels of visit coverage. Ensure weekend rotas are finalised in advance to maintain service stability. Work collaboratively with recruitment colleagues to address staffing gaps and support workforce planning. Training Coordination Identify training requirements in partnership with the Head of Care. Arrange and coordinate internal and external training, including clinical competencies for complex care packages. Verify staff competency prior to deployment. Senior Care Team Collaboration Work closely with Senior Carers and Team Leaders to maintain operational stability. Delegate day-to-day operational matters appropriately while managing escalations relating to risk, family concerns, or service delivery issues. Contribute to team meetings and development initiatives. Quality Assurance and Compliance Ensure compliance with internal policies, regulatory standards, and safeguarding requirements. Maintain accurate and up-to-date operational documentation. Monitor service performance and implement improvements where required. Family Engagement Act as the primary operational contact for families. Respond to concerns promptly and support positive service experiences. Reporting and Data Management Produce regular operational reports relating to service delivery, onboarding timelines, training compliance, and workforce capacity. Maintain accurate data within care management systems. Travel and On-Site Support Travel across the North West region to support care delivery, conduct site visits, and provide operational oversight. On-Call Duties Participate in the on-call rota, responding to urgent operational issues within agreed response timeframes.
