KEY RESPONSIBILITIES To effectively track the patients journey from admission to discharge in a timely manner using in house IT system CDT To attend Multi-Disciplinary/ Best Interest meetings to track and update patient journeys which may involve some travelling across the D2A footprint to assist with the flow of patients through the service. Follow-up any outstanding professional actions identified in the Multi- Disciplinary Team meeting/ with the appropriate professional/services and establish timescale for completion ensuring CDT is kept accurate. Liaison with families and professionals including external agencies to provide timely updates and discharges including the booking of transport. Monitoring the progress of ongoing referrals and assessments escalating any issues to the Patient Flow Coordinator Reporting of planned discharges to internal teams to facilitate timely admissions and discharges from acute settings Develop and manage strong relationships with all partners and colleagues to support timely updates and flow through discharge to assess beds. To be able to work closely and at pace, assisting other members of the team to meet deadlines, sharing workload and providing cover for absences where necessary, attending meetings and updating CDT. Flexible approach demonstrating ability to respond positively to changes in allocation of work at short notice to set deadlines. Systems and equipment Update and maintain accurate information within shared local health economy patient tracking systems. Ensures data and reporting information is presented in a timely manner as required by keeping CDT system updated, adding and discharging patients, uploading documents, ensuring data system is accurate for reporting needs. Use of Excel, Office and Teams Maintains confidentiality in line with MPFT policies and procedures Supports the recording of incidents, safeguarding via MPFT system Decisions and judgements Able to work on your own initiative with supervision, prioritising workload to ensure effectiveness and efficiency in delivering objectives escalating delays and issues when needed. Communication and relationships Adapting communication styles to suit audience with varying cognitive impairments, adhering to trust values using care and compassion when providing and receiving information with both colleagues and patients. Develop and maintain strong relationships with all partners and members of the clinical triage team to support discharge and admission avoidance as the service dictates using verbal and written skills. Manage relationships with and work alongside all partners associated with the Integrated Complex assessment team, the Post holder will be expected to be professional and Respectful working within the trust values. To assist with the induction of new staff and support with all training needs Physical demands of the job Occasional exposure to distressing or emotional circumstances. Frequent requirement for long periods of concentration, with frequent interruption Work on a rota basis to meet service needs Frequent use of a Telephone, VDU and keyboard. Frequent requirement for sitting for long periods of time
