Crynodeb
To assess and manage patients referred to the Ealing Rapid Response Service with complex physical, mental health and social issues. This will include providing specialist therapy assessment and support as directed by senior clinicians to support early discharge from hospital or avoid admission to hospital.
To deliver all elements of Occupational Therapy or Physiotherapy assessment and treatment in a community setting, usually in the patient’s own home or care home in a timely manner.
To supervise, train and support workers and students as directed by Senior Clinicians.
To work collaboratively with other members of the multi-disciplinary team and colleagues in the rapid response service ie medical staff, nursing staff, other therapy staff and support and admin staff as well colleagues in other community services, to improve standards of care and respond to changes in service need.
To provide specialist advice, within yourscope of skill and practice in Occupational Therapy or Physiotherapy management to other colleagues on request and to facilitate the highest possible standards of care and practice within the Intermediate Care Team.
• To share ownership of the Rapid Response / Home First pathway by attending regular MDT meeting, taking responsibility for the caseload & monitoring the service.
• To liaise with medical, nursing and therapy staff, patients, carers and external agencies to ensure the safe and effective care of patients in the care of the Intermediate Care Team as appropriate.
• To ensure as far as is practicable, that the identified patient’s needs are met through co-ordinated planning, the implementation and evaluation of input provided by the Home First Team, involving other staff and disciplines as appropriate.
• To liaise with other team members, outside agencies, patients and carers to plan the safe transition of care from the Intermediate Care Team, when required.
• To complete a comprehensive assessment of patients’ needs in their own home, including environmental and manual handling risk assessments.
• To provide specialised Occupational Therapy or physiotherapy expertise in the assessment treatment and management of patients in their home environment. This will involve high level analytical, reasoning and risk assessment skills to select appropriate interventions from a range of options.
• To provide Occupational Therapy or physiotherapy assessment and treatment in a way that is safe, clinically effective and responsive to changes in service demand and consistent with current best practice.
