Summary

The ideal candidate will work well under pressure, have the ability to work both autonomously as well as part of a team and have excellent written and oral communication skills. The post holder is expected to take charge in the absence of the Sister.

The post holder is expected to take charge in the absence of the Departure Lounge Manager/Deputy manager and is expected to take leadership roles from time to time. The post holder should ideally have some experience in the use of EXCEL but this is not compulsory.

Job Description Summary

The core purpose of the role is to facilitate timely discharges, improve patient flow, and ensure a safe transition for patients leaving the hospital.

Key Responsibilities
• Patient Assessment and Care Planning: Conducting comprehensive nursing assessments, developing, implementing, and evaluating evidence-based programmes of care.
• Discharge Coordination: Coordinating and facilitating patient transfers from wards/departments to the discharge lounge and their eventual destination (home, care facility, etc.). This includes liaising with social services, therapists, transport services, and pharmacy.
• Medication Management: Administering medications as needed and ensuring all discharge medications (TTOs) are received and understood by the patient/carer.
• Communication: Acting as the primary communication link for patients, families, and the wider multidisciplinary team, providing information and addressing concerns regarding the discharge process.
• Environment Management: Ensuring a safe and comfortable environment for patients in the lounge, which may operate during varying hours including weekends.
• Leadership/Supervision: Taking charge of the lounge when required and supervising junior staff, healthcare assistants, and student learners.
