Summary
We are looking for a Band 4 Community Nurse Associate to work within our Isle of Ely Community Nursing team supporting patients with clinical health needs.
As a nursing associate you will provide high standard of nursing care and interventions within the patient’s own home, with support from the wider team. The patient will be at the centre of the care being provided, ensuring patient and carer participation in decision making. Our area covers Ely and surrounding villages.
We provide a service seven days a week, working closely with other community services and PCN's to provide high standard care.
You will have the opportunity to develop yourself, with extensive learning and development opportunities.
You will be working as a member of the Isle of Ely Community Nursing Team undertaking delegated duties in order to meet the needs of the needs of the service users. You will be trained and supervised by registered professionals and will be expected to follow a plan of care based on the specific needs of the service users.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines.You will need to hold a full UK driving licence*and have use of a vehicle. We are ideally looking for people who have:
• Experience of community nursing, palliative care, care of patients with diabetes
• Educated to Registered Nursing Associate on the NMC Register
• Experience in falls prevention work or willing to gain skill
• Experience of caring for and supporting adults and older people
• Experience of assessing patients and formulating care plans independently and where appropriate, with support from RNs in the team.
• The ability to demonstrate a commitment to high quality service provision
• The ability to work flexibly and as part of a team, as well as individually
• The ability to travel independently using own vehicle
• Good time management and the ability to manage individual workload
• Enhanced communication and inter personal skills

1. To be responsible for the assessment, planning, implementation and evaluation of the evidence based care required, including health promotion for a named service user/group ensuring it meets Trust timescales in line with agreed NMC standards.

8. To demonstrate care and compassion in your work.
9. To respect and value the different therapeutic approaches within the multidisciplinary team but ensure that the agreed philosophy approach is adopted and followed.
