We are now seeking an experienced Care Coordinator to join our team — a pivotal leadership role providing clinical oversight and care governance across our supported living services. The Role
As Care Coordinator, you will play a central role in ensuring safe, high‑quality, person‑centred care across our services. You will work closely with operational and quality teams, offering clinical leadership and supporting best practice in line with CQC standards.

What We’re Looking For
Essential
NMC Registered Nurse (RMN, RNLD, or RGN with community/complex needs experience).

Minimum 3 years’ post‑registration experience in a clinical or care‑coordination role.

Experience in supported living, residential care, or community adult health.

Strong understanding of CQC, DoLS, MCA, and risk management frameworks.

Excellent communication, leadership, and organisational skills.

Ability to travel between services (mileage reimbursed).

Highly Desirable
Experience with forensic clients or dual diagnosis.

Experience supporting services through CQC inspections.
