Title:Nurse or Paramedic Streamer (Adults and Children) Specialty:Queen Mary Sidcup Urgent Care and Out of Hours Service Location:Queen Mary Sidcup Urgent Care and Erith Hospital Employment type: Permanent Work hours: Flexible Salary: Band 6 (depending on skills and experience) Accountable and Responsible to:Emergency Clinical Practitioner and Lead Nurse Key Working Relationships Key relationships include but are not limited to: BHNC Board BHNC Senior Management Team UTC Senior Management BHNC Training Hub Key Responsibilities Duties The postholder will be responsible for ensuring the delivery of safe and effective nursing care to the Urgent Care patients. The streamer will be involved in dealing with the initial assessment of all (adults and children) ambulatory adult cases arriving at the Urgent Treatment centre. Make autonomous decisions on directing patients to the correct clinical area of service. Be professionally and legally accountable for all aspects of their professional and clinical work within agreed national, local and professional standards and guidelines. As a clinical streamer, they are responsible for the care delivered during the streaming / triage process. The streamer demonstrates critical thinking and skills in clinical decision-making. They will work collaboratively with the whole Urgent Care team to meet the needs of patients, supporting the delivery of policy and procedures. Support patients using the streaming and redirection tool at the first point of contact. Identifyusing knowledge andexperience,any patients who present with red flag symptoms and escalate effectively. Recognise and work within own competence and professional code of conduct as regulated by the NMC regarding wound care, medication administration and observation monitoring. Provides medication administration under Patient Group Directions and will follow the necessary legal duties, training and documentation mandated. To use enhanced skills and knowledge to assess the physical and psychosocial needs and develop a comprehensiveand concordant plan of care with the patient whenrequired. Observe,recordand report all changes to a senior clinicianin a timely manner, depending on acuity and urgency. Produceaccurate,contemporaneousand complete records of patient consultation, consistent with legislation,policiesand procedures. Prioritise,organiseand manage ownworkload in a manner thatmaintainsand promotes quality,ensure effective time-management strategies are embedded within the culture of the teamusingall available resources. Deliver care as an individual and team according to the National Framework 2022(gov.uk),NICE guidelines and evidence-based care. Support Emergency Clinical Practitioners, Advanced Clinical Practitioners and Urgent Care GPs, with caseload in minor injuries/illness. Excellent decision-making skills under pressure. Give accurate and comprehensive hand over to team members at each shift change, ensuring that pertinent information, consideration, acute presentations etc are fully disseminated to colleagues. Provide health education and promote healthy lifestyle for patients and carers. Provide clinical and professional advice and support to the team. Promote safe systems of work andidentifyareas where changes in the Urgent Care would achieve a more effective outcome. Ensure resources are used efficiently and effectively and all equipment used in the care of patients is appropriatelymaintainedand used only by those individuals with theappropriate training. Work with patientsin order tosupport compliance with and adherence to prescribed treatments. Actively challenges unacceptable behaviour and discrimination. Prioritisehealth problems and intervene appropriately toassistthe patient in complex, urgent or emergency situations, including initiation of effective emergency care. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice. Understand own role and scope andidentifyhow this may develop over time. Liaise with other departments,members of the Bexley Health Neighbourhood Care and other organisations to ensure the smooth running of the role toassistthe functioning of the team.
