As part of the Therapy Hub, you will be responsible for maximising the independence and autonomy of people, ensuring that interventions are provided fairly and in a manner that is sensitive to individual needs. You will have good assessment and treatment planning skills, an ability to prioritise workload and enjoy a multi-disciplinary rehabilitation approach. You will work in partnership with the Early Intervention Community Team to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service.



1. To work in accordance with the Royal College of Occupational Therapy (RCOT) ‘Code Ethics/ Professional Conduct’, and the Health and Care Professions Council (HCPC) ‘Standards of Proficiency’.

2. To be responsible for the provision of Occupational Therapy within the Division in a variety of settings.

3. To be responsible for carrying and prioritising own caseload and assisting junior staff, Rehabilitation Technicians and Rehabilitation Assistants to manage their caseloads ensuring effective handover of caseloads during periods of absence.

4. To actively contribute in the promotion and development of Falls Awareness, in conjunction with partner agencies raising general awareness within the community with the aim of reducing falls risk factors.

5. To ensure, where possible, that all intervention is evidence based, utilising appropriate outcome measures when applicable.

6. To assist in setting professional and service standards and participate in the auditing of these standards.

7. To be professionally accountable and responsible for all aspects of own work, including the generic and or specialist assessment of clients with a variety of conditions, ascertaining appropriate information, analysing their physical, social and psychological needs and sign posting to the appropriate professionals within the team and external agencies.
