An exciting opportunity is available within Adult Care, Housing and Public Health for a qualified Social Worker in the IDT (Integrated Discharge Team)- Transfer of Care Hub

We are committed to providing adult care and integrated services that put the person at the centre of our work. We are passionate about following the ‘Home First’ pathway and work closely with our hospital colleagues to achieve this.
We cover a range of areas such as intermediate care bed bases and supporting discharges from out of area hospitals. We work collaboratively with health partners in the Transfer of Care Hub and complete Care Act assessments for people accessing Integrated Rapid Response.
This is an exciting time to join the team with the development of our Transfer of Care Hub, you will be an integral part of MDT working for hospital discharges and progression of D2A pathways.
Successful candidates will join a supportive team which is comprised of health and social work professionals based at Woodside. As part of this role, you will be expected to support others in the team, including Assessment and Review Coordinators to develop their skills and experience.
A requirement of the post holder will be availability for weekend working on a rota basis.
